either Doug Becker or Deborah Havens (Doug's wife).
I commit by this notice to pay in full any medical fees that
are deemed necessary.
I understand that Doug Becker is an experienced and
caring dog trainer, however, there is always a chance of
mishap. Therefore, I hereby release Doug Becker and his
agents from any and all liability of any nature, including
injury or sickness to me, my family, or my dog. I expressly
assume the risk of any damage or injury caused by my dog
while being trained or boarded by Doug Becker.
